The Atlanta Huaqiang Soccer Club will not assume responsibility for any injuries incurred while participating in the HQ League. In addition, I state that I am in proper physical condition for this soccer league and that I have medical coverage with an insurance company while participating the HQ League. I, the undersigned for myself, my heirs and assigns, do hereby release the Atlanta Huaqiang Soccer Club from any and all claims for injury, death, loss or damage I may suffer as a result of my participation.
